Welcome to DecideACT
The first step to joining the study is to find out if you are eligible. To do that, we need you to take a short (about 10 minutes) screening survey. It has some basic questions about you and your smoking. First, we want to give you a little bit of information about DecideACT:

DecideACT to test the efficacy of a quit smoking program for Hispanic adults

You are invited to take part in a research study. The goal of this study is to find out which quit-smoking programs work best for Hispanic adults who want to quit smoking. The DecideACT study is being led by researchers at Fred Hutch Cancer Center.
This study is for Hispanic adults who smoke cigarettes and want to quit. If you join the study, you will be asked to download and use the DecideACT smartphone application (“app”). There is no cost s nothing to join this study.
We will also ask you to complete follow-up surveys about your experience with the program. You will receive surveys at 3, 6, and 12 months after joining the study. You will also receive a short 2-question survey 6 weeks after joining.
You will receive $25 for each completed 3-, 6-, and 12-month survey (up to $75). If you complete one of these surveys online within 24 hours of receiving it, you will receive an extra $10 bonus for that survey (up to $30 total in bonuses). You will also receive $5 for completing the short 6-week survey. If you are eligible and join the study, you may earn up to $110 by completing all the follow-up surveys. If you are selected to also complete a saliva test at 12 months to detect cigarette smoking, you would receive an additional $35, so you may earn up to $145 total.
If you are assigned to receive them, you may get free nicotine patches, gum, or lozenges to help you quit smoking. Not everyone in the study will receive these products.
To find out if you can join the study, please complete a short screening survey. This survey asks basic questions about your smoking history, age, race, where you live, and your gender. Your answers will be entered into a secure website and kept private by the study team.

You do not have to be in this study. You are free to say yes or no, or to drop out after joining. There is no penalty or loss of benefits if you say no.

Do you consent to complete the screening survey? The survey takes about 10 minutes. There is no payment for completing this survey.
